Students are required to submit the application on or before the end of the first week of the semester they expect to complete their degree requirements. This will assure the listing of names in the commencement program. 

If a student expects to complete their degree requirements during summer semester, submit this application on or before the end of the first week of spring semester. This will assure your name will be listed in the spring commencement program. 

For students applying to confer their Master of Public Health, fill out the  online graduation application form.

For students applying for a previous semester, print and complete the  hard copy graduation application form and fax or mail it to the Office of the Registrar.

For students applying to confer a graduate certificate, print and complete the Graduate Certificate Application form and fax or mail it to the Office of the Registrar.

State Authorization: Distance Education Restrictions Based on Permanent Residence

Michigan State University is obligated to comply with existing state laws regarding distance education (programs/departments offering courses delivered exclusively online). State laws regarding distance education vary state-by-state, and even program-by-program within a state. The Master of Public Health offered through the College of Human Medicine’s Division of Public Health at Michigan State University requires a practicum experience that constitutes a physical presence in some states, therefore restricting enrollment in our program to residents of those states.
